We have done our best to simplify the Allen County Permitting Process. Below is a form to submit all necessary information. Please fill out in it’s entirety. You will need to attach your Government ID and BBP certificate.

These responses will be used for us to complete your guest artist permit and hepatitis B vaccine declination.

Once your information is submitted a confirmation message will appear with a link to pay the $50 Guest Artist Fee.

Artists already permitted in Allen County can upload a copy of their current Artist Permit.
